When sending samples to the lab:

1. Please ensure that each individual sample is labeled and sealed properly.

Make sure the analysis request form has been filled in compeltely and is enclosed within water tight packaging.

2. Download and complete appropriate Analysis Request Form. Please include the request form along with any other required documents (i.e. shipping papers, photos, collection forms, FDA letter and test request form for an FDA detained shipment) securely in the box with samples. This will ensure quick processing and help get results reported to you in a timely manner. Add dry ice or blue ice in the box for preservation if needed.

3. If sending multiple samples, keep samples in separate containers.

    a. Separate samples by placing them in individual bags or containers
    properly labeled to prevent cross contamination (i.e. different lots, different day
    codes, different subsamples. Include a control sample if required.)

Samples should be kept separated so as to prevent cross contamination.

    b. When submitting multiple samples in the same box, please secure the ADPEN request
    form in a separate plastic bag inside the box.



4. If multiple samples need to be combined and homogenized into one, please clearly indicate which samples are to be combined in writing in the analysis request form. Use additional pages if needed. Make sure that combining sample lots is acceptable to any Agency involved. Contact ADPEN client support team for guidance on collection and documentation questions.

6. Once samples have been analyzed, your results will be provided to any Agency involved and/or faxed or e-mailed to the contact listed on the analysis request form.
